Follow one referral, from a GP's letter to a first appointment

A cardiology referral at a fictional health group. Six screens, three people and one AI agent with its own purpose. Scroll, and the screen follows the step.

Sample organisation: Ardmore HealthPeople: GP, consultant, booking teamTime: received to booked in one morning

Step by step

  1. 01 · Referral

    A GP sends a referral.

Dr Keane refers a patient with new breathlessness. The pack files the referral and records the purpose: direct care, triage for cardiology.

Dr Ruth Keane, GP, sends the referral.

  2. 02 · Summary

A summary that cites the notes.

The referral assistant reads the letter, the ECG report and the medicine list, under its own "triage summary" purpose, and drafts a summary that links every line.

The AI agent's access is logged like anyone else's.

  3. 03 · Complete?

    One item is missing.

The local protocol asks for an echo if available. It has not been done, so the pack flags it before the consultant reads the case.

The referral is not sent back for one missing item.

  4. 04 · Consultant

    The consultant sets the priority.

The protocol suggests urgent. Dr Lynch agrees, sets the priority and asks for the echo before clinic. The suggestion is shown as support, not as a decision.

Dr Sarah Lynch, consultant cardiologist, decides.

  5. 05 · Booked

    Booked, without the clinical detail.

The booking team sees the name, contact and priority, and books clinic and echo. They never see the summary.

Tomás Reyes, booking team, books both.

  6. 06 · Sealed

    Sealed with every access.

The decision, the summary, its sources and every access to the record are sealed together. The GP and the patient are told.

Information governance can check who saw what, and why.

The record at the end

One record for the whole referral.

The referral ends with one sealed record: the decision, the clinician, the summary and its sources, and every access with its purpose.
